NEW PATIENT FORMS If you are a new patient then we are very happy that you decided to contact us and we are excited to meet with you. For your convenience, the four new patient forms may be downloaded and printed off from your computer. Please bring the completed forms with you to your initial appointment with Dr Benson. You may also feel free to mail us the forms prior to your first appointment so that we may be prepared in advance of your arrival. If you are transferring your dental records, including x-rays, please be sure to print off, complete, and submit your 'Authorization To Release Dental Records' form to your previous dental office well in advance of your appointment with Dr Benson. This will give your previous dental office the the appropriate amount of time to provide documents, such as x-rays, that are important for your discussions with Dr Benson.

This includes photos of yourself or others, if appropriate, that show your teeth as to how you would like Dr Benson to restore your smile.
